MediPet South Africa

First up: I did not design the MediPet website. Just so there's no confusion - I have merely designed and implemented a part of their customer forms. 

MediPet is going through some changes right now and, as part of a system refresh, needed an update on their Add-A-Pet policy form. Using the Chronoforms tool for WordPress I created a responsive form filled with custom features. This form allows existing MediPet customers to add a new family companion to their insurance policy in a clear, easy-to-follow manner.

The form includes a raft of customised CSS and Semantic-UI class implementations, custom jQuery functions and AJAX calls, and various complex PHP verification routines.
